Practice Partner Helps Clinic Manage Pay-Far-Performance Program

THE PRACTICE

South Park Internal Medicine is a 2 physician, Internal Medicine practice serving the community of Highlands Ranch, Colorado.

THE PROJECT

When Anthem Blue Cross and Blue Shield of Colorado started a pay for performance program, the physicians at South Park Internal Medicine jumped at the chance for additional reimbursement. The pay for performance program is based on ten clinical parameters including screening for preventative measure such as mammography, PSA, hypertension control, HgA1C, and Pap smear.

South Park was able to easily adjust Practice Partner's built in age, sex, and disease specific health maintenance reminders to specifically target the performance parameters required by the carried.

With Practice Partner's unique progress templates combined with an electronic lab interface that automatically imports lab data into the patient chart, the physicians were able to incorporate the quality measures into their daily workflow. For instance:

  • Most recent HgbA1c results are automatically presented to the physicians while documenting their visits with diabetic patients.

  • Reminders for overdue preventative screening measures such as PSA are automatically presented to the physicians during documentation of the patient visit

THE RESULTS

The clinic met or exceeded 8 of the 10 quality targets, resulting in additional reimbursement

CONCLUSION

South Park Internal Medicine was able to take advantage of Practice Partners built in quality improvement tools to develop a system that allowed the physicians to follow recognized standards of care, without disrupting their work flow or productivity.
